Flare (microdose lupron) Protocol vs Lupron (long lupron) vs Antagonist Protocol

Today we went to see another reproductive endocrinology (RE) our friend recommends. It was our first meeting with him. Something we have learned so far;

We already learned from Dr. Wong (1) that Lupron helps synchronous eggs and make them progress at the same rate and mature at the same time. The downside is it can suppress the ovary too much that no eggs or less eggs will be released when trigger (hcg)

The Antagon/antagonist protocol (we had) can give more follicles/eggs. But they are not at the same stage (some mature, some aren't). Thus, can result in only a few mature eggs.

This RE doctor (Dr Wong (2)) suggests Flare protocol. The protocol uses Lupron in microdose with the hope that it will not suppress ovalry too much to preven it from ovulation.

High FSH Level

Yesterday we went to see our doctor for a post-IVF consultation. Now we know why Nui needed a lot of Gonal-F (stim med) to stimulate the eggs. Last cycle she had more than 6075iu of Gonal-f (FSH) and 825iu of Menopur (FSH+LH). And we got 9 follicles.

Doctor told us Nui has high FSH level (normally it means Day 3 FSH is higher than 10).

Below is her history FSH level. It is interesting that we do not have the FSH level data from the IVF cycle we just had.

Oct 08 = 15.2

FSH is a hormone, produced by pituitary gland, to stimulate ovary to produce eggs. From what I have read so far, high FSH means the ovary does not response well to FSH, thus human body automatically generate more FSH (by pituitary gland) trying to stimulate eggs. Some say that to fix this we need better blood flow to ovary and uterus. Some also say it may cause from stress. High FSH level should affect to quantity of eggs only, it should not have affect to quality of eggs.

Last night we did some research to find what we can do about it. It seems acupuncture and herbal medicine could be helpful. Many people on internet who has similar problem suggest to read the infertility cure book. เช้านี้หมอโทรมาบอกได้ไข่ 9 ใบ ปฎิสนธิไปแล้วสองใบ เนื่องจากมีแค่สอง หมอคิดว่าจะให้ถ่ายตัวอ่อนเข้าท้องนุ้ยตอนวันที่ 3 ก็คือวันพุธนี้ เวลา 10 โมงเช้า

พรุ่งนี้อาจจะมีจำนวนตัวอ่อนมากขึ้น หมอจะโทรมาอีกทีตอนเช้า

เอาไข่ออกมาแล้ว

วันนี้นุ้ยไปเอาไข่ออกมาแล้ว หมอนัด 7โมงเช้า ไปถึงก่อนเวลานิดหน่อยประตูตึกยังไม่เปิด ต้องโทรไปขอกเจ้าหน้าที่ร้านหมดให้ลงมาเปิดให้ เอาเช็คให้เจ้าหน้าที่เป็นค่าหมอดมยา $400

หมดดมยามาประมาณ 7:15 ก็บอกให้นุ้ยไปเข้าห้องน้ำให้เรียบร้อยแล้วก็พาไปห้อง จัดแจง
เอาเครื่องตรวจวัดต่างๆ ติดให้พร้อม ใส่น้ำเกลือ ใช้เข็มเล็ก หมอบอกที่โรงพยายาลไม่มีขนาดเล็กเท่านี้แต่ที่ร้านหมอวองมี นุ้ยบอกไม่เจ็บเลย
สักพักหมอวองก็มา หมอดมยาก็เลยบอกให้เราออกไปได้

สักชั่วโมงนางพยาบาลก็มาตามให้ไปดูนุ้ยได้ นุ้ยถูกเข็นมาไว้ห้องเล็กที่อยู่ติดกัน กำลังเบลอๆ นางพยาบาลไม่อยากให้นุ้ยนอน ก็พยายามชวนคุยโน่นนี่ เขาบอกว่าบางคนเจ็บ ปวด ต้องกิน Tylenol.
สัก 10 โมงนุ้ยก็เดินได้ พวกเราก็กลับบ้านกัน แวะร้าน Cream Pan ซื้อขนมและเค็กมากิน

นุ้ยไม่เจ็บปวดใดๆเลย กลับมาชงโปรตีนให้กิน นุ้ยดูเมล์ คุยกับแม่ของริน แล้วก็ไปนอน ตื่นอีกทีประมาณบ่าย 4

หมอบอกว่าได้ไข่มากกว่าที่คิดแต่ยังบอกจำนวนที่แน่นอนไม่ได้เพราะต้องดูผ่านกล้องอีกที
พรุ่งนี้หมอจะโทรมาบอกว่าได้กี่ใบ
หลังจากนั้นอีกวันจะรู้ว่าปฏิสนธิกี่ใบ

hCG Trigger

Finally, it is the time for hCG trigger that means we no longer need stim (Gonal-f) nor Meds (Menopur). Wow, we end up need more Gonal-f and menopur than what we expected.

The hCG trigger was given to Nui today at 7:30PM. Egg Retrieval (ER) will be 36 hours after the trigger. So, the ER will be performed on Sunday at 7:30AM.

Tomorrow Nui has to start taking Doxycycline two time a day for 7 days.

วันนี้ฉีดยากระตุ้นเพื่อให้ไข่ตก ตอน 1 ทุ่มครึ่ง หลังจากนี้อีก 36 ชั่วโมงจึงจะเก็บไข่ เพราะฉะนั้นจึงต้องไปหาหมอวันอาทิตย์นี้เวลา 7 โมงเช้าเพื่อทำการเก็บไข่

Doctor needs 15 healthy eggs. Nui has not produced enough, so the Egg Retrieval day is postponed. It will be either this Friday or Sat instead of planned date, Thursday Jul 23...
umm what about HCG trigger then. Maybe no need to trigger it anymore???

Tomorrow Nui has to go see the doctor at 8AM! And more gonal-f has been ordered.
So far she has been injected with 6075iu of Gonal-f and 825iu of Menopur.

Need more Gonal-f

Today Nui went to see the doctor again. We need 3 more boxes of the Gonal-f 600iu.
So, the doctor office placed order for us. It will be delivered tomorrow.

Increase dosage

Yesterday Nui went to see the doctor. She does not response well with the medication or maybe the reponse is slow. So, doctor increase the dosage of the gonal-f to 600 iu/day instead of 450 iu/day
So, on Wed Nui inject herself with 600 iu in the evening.

She will have to see the doctor again this Friday.

First injection

ใช้เข็มเดิม เจาะขวดไปสองที เอาไปจิ้มพุงนุ้ย ครั้งแรกไม่เข้า สงสัยจิ้มช้าไปหรือเข็มสึกหว่า
จิ้มอีกทีเร็วหน่อยเหมือน instruction ว่าให้เหมือนปาเป้า ก็เข้าดี พรุ้งนี้เปลี่ยนเข็มสองครั้งดีกว่า จะได้คมๆหน่อย Gonal-f หนึ่งกล่องให้มาตั้ง 6 เข็มอยู่แล้ว

ตกลงเข็มแรกฉีดประมาณ 8:02AM
เดี๋ยวเย็นนี้ต้องฉีด gonal-f 225iu อีกทีกะว่าตอน 4:00PM
หมอบอกฉีดเช้าทีเย็นที ห่างกันเท่าไรก็ได้ ให้ฉีดทุกวันเวลาเดิม
Dose นี้ให้ฉีดถึงวันอังคาร พุธเช้าไม่ต้องฉีด นุ้ยต้องไปหาหมอ 10:15AM วันพุธ เดี๋ยวหมอดูอีกทีว่าต้องเปลี่ยน dose หรือเปล่า

Post-ops

วันนี้นุ้ยไปหาหมอมา หมอก็ ultra-sound บอกทุกอย่างปกติ ดีหมด เริ่มได้เลยไม่ต้องรอ ก็เลยสั่งยาเลย สรุปตั้ง $4000 เกือบสูงสุดที่ 5000

ตกลงบอกหมอว่าเริ่มเดือนนี้เลย วันนี้หมอก็เลยให้สั่งยาไป $4000 เหรียญ

วันนี้ นุ้ยไปผ่าตัดเอา ติ่ง เล็กๆ polyps ออกเพื่อให้มดลูก ราบเรียบสวยงามพร้อมให้ตัวอ่อนฝัง

ความจริงหมอบอกว่ามันเล็กมากจนไม่ significant แต่พวกเราตัดสินใจเอาออกดีกว่า
พรุ่งนี้นัดหมอ ตอนบ่ายโมงครึ่งเพื่อ follow up - post ops
ระหว่างทางกลับกินขนมปัง 1 ก้อน กลับมาบ้านได้สักพักอวกออกมาหมด
รอไปสักพัก ชงโปรตีน 1 scoop ให้กิน ท่าทางจะโอเค

เช้านี้ไปทำ semen analysis มา เผื่อมันดีอาจจะทำแค่ IUI ฮ่าๆ
ไว้อีก 2-3 วันจะโทรหาหมอฟังผล
